1) Please collect your consent forms from Ms Siti's pigeon hole outside the staff room by Monday 19 March. PL Russell please ensure that all those who are going have taken the consent form by Monday. Please collect the consent forms latest by Wed 21 Mar and please personally pass all the consent forms to me on 21 Mar (Wed).
